In the first, scissors are shown cutting through adhesions between the uterus and bowels, and it is suggested that the scissors are causing direct damage to the bowel. In the second, a retractor is shown pulling the adhered bowel away from the uterus, and it is suggested that the retraction is causing indirect damage to the bowel.
