Correct vs Incorrect Placement of Trocar into Abdomen

There are 3 rows of illustrations on this case: the first top image is a female placed in the left lateral decubitus position with the abdomen insufflated using a Veress needle. The second row, are images showing a safer placement of the first trocar through the upper right quadrant in this case, versus entry through the umbilicus. The third row of images depict placement of the trocar through the umbilicus with just the tip of the trocar below the peritoneum where it should have stopped, followed by placement of the trocar in a more steep angle and placed too deep into the retroperitoneal space injuring the aorta and vena cava.
